Job Description


Hope Solutions seeks an energetic Clinical Support Services Manager who is flexible and responsive to support the needs of our clients while possessing the skill set to collaborate with interdisciplinary team members and community partners. We are looking for compassionate service-driven individuals who fundamentally believe in the potential of others. The Site-Based Clinical Support Services Manager (CSCM) is a Hope Solutions leadership team member responsible for clinical and case management services delivered to families and individuals at site-based housing settings. The CSCM is responsible for supervising site-based staff, including supervising case managers in the Los Medanos/Bella Monte site, the Lakeside site, and the Hacienda site. The CSCM is the lead at their sites and will collaborate with other service providers and property management as part of a multi-disciplinary team. The CSCM will also collaborate with the training team and the HR Director to design and deliver clinical training for the Hope Solutions staff.

Supervisory Responsibility: Los Medanos/Bella Monte Case Manager, Lakeside Case Manager, Hacienda Case Manager


Leadership Role:

  • Active participation and contribution to the Management Team of Hope Solutions.
  • Cultivate relationships with key stakeholders and County leadership.
  • Assume DSCS responsibilities when they are away.
  • Represent Hope Solutions by attending relevant community events, including providing presentations regarding the Hope Solutions service model and impact.
  • Represent Hope Solutions with community stakeholders as assigned. May require evening meetings.
  • Attend monthly Hope Solutions Board Meetings as directed.
  • Work with the training team and HR Director to develop an agency-wide training schedule of essential training to prepare staff to deliver the highest quality of service.
  • In collaboration with the DSCS, develop annual logic model output and outcome goals for supervised programs. Prepare annual and semi-annual reports of logic model outcomes.

Clinical Services:

  • Working with the Director of Site-based Clinical Services (DSCS), develop and monitor standards of practice and outcome measures for clinical and case management services.
  • Working with the DSCS, develop a training program for agency staff to support and inform quality care.
  • Follow the EPSDT documentation structure consistent with contract obligations, standards of practice, and outcome measures. Ensure that these are strictly adhered to by case management and clinical staff.
  • Develop an effective Quality Assurance system for reviewing EPSDT notes and files.
  • Work with the DSCS to implement evidence-based and continuous quality improvement practices across all clinical and case management programming serving families and individuals.
  • In collaboration with the DSCS, act as agency liaison for county-related contract obligations, county meetings, Homeless Management Information System (HMIS), EPSDT, and behavioral health initiatives which involve the housing programs of Hope Solutions.
  • Ensure compliance and oversight of all contract compliance issues such as HIPPA, confidentiality, and professional conduct of all supervised staff.
  • In collaboration with the Director of Finance and the DSCS, ensure compliance with county service contracts.
  • Develop protocols and oversee training and certification for emergency response, CPR, and first aid for all family case management/mental health staff.
  • In collaboration with the DCSC, manage budgets for supportive services programs for residents.
  • Provide coverage for DSCS as needed.

Contracts and Funding:

  • In collaboration with the DCSC and Leadership Team, assist in preparing county and clinical service-related applications for funding and semi-annual grant outcome reports.  
  • Oversee billing, audits, utilization review, chart quality, and all requirements associated with the EPSDT program.

Organizational/Administrative Responsibilities:

  • Professionally represent Hope Solutions in all circumstances.
  • Comply with policies, procedures, standards of practice, and outcome requirements for the program.
  • Comply with all legal/ethical professional guidelines for maintaining consumer confidentiality, protecting consumer rights, advocating on consumers’ behalf, and assuring consumer safety.
  • Comply with all local, state, and federal regulations and Hope Solutions Policies and Procedures.
  • Maintain knowledge of and compliance with Fair Housing regulations.
  • Maintain and supervise maintenance of confidential data files that adhere to HIPAA regulations.
  • Assist with the creation/distribution of client satisfaction/feedback surveys.
  • Assist with data entry and completion of financial reports, tenant statements, billings, and correspondence.
  • Attend all required meetings, including but not limited to: Hope Solutions staff meetings, program staff meetings, consumer case conferences, and linkage meetings with other agencies.
  • Complete required personnel-related paperwork and complete expenditure reports for service expenses and/or mileage in a timely and accurate manner.
  • On-call, after-hours, and backup work will be required.
  • Other duties as assigned.

Position Qualifications:

  • Passion for and demonstrated experience successfully supporting vulnerable populations, especially homeless families or individuals with mental disabilities.
  • Knowledge of and belief in “Housing First” philosophy and strategies.
  • Master’s degree in Social Work/Marriage and Family Therapy/Professional Clinical Counseling preferred.
  • Licensed or license eligible toward LCSW or LMFT preferred.
  • Substitution of experience for education as follows: B.A. Degree and/or six years of work in the field, including two years of experience providing program management, budget management, and staff supervision/oversight in behavioral health services.
  • Bi-lingual in English/Spanish preferred.
  • Outstanding written and verbal communication skills.
  • Computer proficiency in the use of Microsoft and database applications.
  • Must have operational vehicle, auto insurance, valid California driver’s license, and be willing to drive for work.
  • Hope Solutions requires all staff members to either be fully vaccinated (including a booster within six months of the last vaccination) or complete weekly testing.
  • Must pass LiveScan screening and TB test.
  • Salary contingent upon skill and experience.

Physical Qualifications:

  • The person in this position may make home visits to residents who live on second or third-floor apartments (without elevators) and will need to be able to climb stairs.
  • May accompany residents on walks to local resources (grocery store, childcare, parks) up to ½ mile in distance.
  • Must be able to work in a desk setting for up to 3 hours to make calls to residents, research resources on the internet, and collaborate with community providers with calls/emails.
  • Office setting activities are required, such as typing on a laptop/computer to respond to emails, entering case notes into a database, and copying/faxing forms and documents on a copy machine related to maintaining or obtaining housing.
  • Occasionally must be able to lift and carry objects weighing up to 25 pounds, including furnishings for new residents, files, holiday gifts, and food donations.
  • Ability to drive an automobile and transport oneself and residents to appointments or meet at residents' homes, including driving at night.
