About Us Health Care For All (HCFA) is a state-based consumer health advocacy organization founded in 1985. HCFA’s mission is to advocate for health justice in Massachusetts by working to promote health equity and ensure coverage and access for all. The organization is recognized as a national leader in its work. HCFA uses three strategies to conduct its work: direct service, community engagement, and policy and advocacy campaigns. HCFA helps tens of thousands of individuals and families in five languages enroll in and renew their health insurance coverage each year and troubleshoot health coverage issues through a unique, multi-lingual consumer assistance HelpLine. Southeast Healthcare has been awarded a new grant in collaboration with The City of Columbus to strengthen outreach to unsheltered residents with the highest needs, with repeated interactions with first responders who often experience mental health and/or substance use challenges. This initiative supports a broader strategy to stabilize housing for all residents and reduce strain on emergency and outreach systems. Responsibilities: Conduct outreach and engagement with referred unsheltered residents. Work in coordination with city staff and first responders to reach city residents. Identify goals and assist with barrier elimination to stable living. Provide services and/or referrals to temporary housing/lodging programs. Maintain accurate and complete records. 1st, 2nd, weekend shifts available
